Office Policies

 

All fees are subject to change without notice.

Cancellation Policy:

We require 24 hour notice for all cancellations.  Any appointments cancelled after 24 hours will be charged a $25.00 cancellation fee.

All No-Show appointments will be charged the full appointment fee.  No Exceptions.




Payments:

All payments are due at the time of visit.  We will accept checks or cash.  At this time, we do not accept credit cards, but this option will be available soon.

Any checks returned for non-payment for any reason will be subject to a $25.00 service fee.

Our office does not accept any form of insurance for payment of services.  We will issue a receipt/super bill that you may send to your insurance company.  Depending on the type of insurance you have, your insurance company may reimburse you for your visit.
